If you are coming to Summit Physical Therapy for the first time, please read below to learn what to expect, what to bring, and what to wear.
What to Expect on Your First Visit
Please arrive 15 minutes prior to your appointment to complete your paperwork. Your evaluation and treatment session will last one hour. During this time, Paul Kane, PT, CMP, will perform a complete evaluation of your condition and begin treatment. Paul will be with you throughout the appointment, guiding you on any special equipment and explaining exercises for you to do at home.
What to Wear
Wear loose, comfortable clothing that will allow for easy examination of the problem area of your body. If the trouble spot is above your waist, consider wearing a T-shirt or tank top. If it’s below your waist, yoga pants or sweat pants will work well. If the problem is located at or below your knees, shorts would be ideal.
What to Bring
Please bring your doctor’s physical therapy referral along with your insurance or medical card or membership information. If you are a new patient or have not been seen by us within the past 12 months, we’ll ask that you print out, complete and bring to your first appointment the following registration forms:
